What to expect:
To complete the automated identity proofing process, we will need to verify a government issued ID that has been issued to you. You will need to enter your name, address and birthdate along with your government issued Social Security Number. The information you submit during this automated proofing process is handled in a secure manner, both in transmission and at rest.
Additionally, we will need to contact you through a mobile phone that you own. If your mobile phone number cannot be verified, you will be prompted to enter a personal credit card to demonstrate control of a financial account instead. This credit card will not be charged any fees during this proofing phase. Later if payment is required, your preferred payment method can be entered during checkout.
Direct Secure Messaging is a federated form of health information exchange supported by the DirectTrust. The DirectTrust requires that all users of Direct Secure Messaging are identity proofed to NIST 800-63-2 Level of Assurance 3. These requirements are not unique to MaxMD and the process is strict by design to prevent cyber impersonation. NIST 800-63-2 identity proofing requires verification of a government issued ID (SSN) plus demonstration of ownership of a utility (mobile phone) or financial (credit card) account. This creates a framework where all users can securely share protected health information with known and trusted counterparties with the assurance that other users have met the exact same identity proofing requirements.
If you prefer an offline identity proofing process or if the automated proofing process is not successful, you can fulfill the identity proofing requirements by completing and submitting this ID Proofing Form.
